Did you resolved your problem in the meantime? I tried going to your website from two different computers and tried different browsers and the form showed up on first load on both of them in every browser.

The form is still not loading on anyone's computer, phone, lap top, etc. we've had our staff try it from different browsers and ip addresses. It only shows up after we refresh the page. How can this get fixed? We plan to send out a mass email tomorrow telling thousands of people to go to the page to fill out the form, but we don't want to do it when it is only randomly half working. We can postpone the email announcement until Friday but beyond that we may have to start looking at other form options that don't cause us loading issues every time.
