Form doesn't always load on wp website

Hello, I just put one of my forms into my word press website (hosted through godaddy), and whenever I go to the webpage its on it doesn't load the first time. I have to hit the refresh button in order for it to come up. I really need it to come up the first time so my clients don't have trouble locating the form. I have tried viewing the webpage on Safari, Chrome, and Internet Explorer, from multiple computers. It happens the same on all of them. I tried inserting the form using your plugin and also embedded it with your JavaScript code. Can you please assist me? The website page is http://circusstarusa.com/compete/

Did you resolved your problem in the meantime? I tried going to your website from two different computers and tried different browsers and the form showed up on first load on both of them in every browser.

Hello, no we have not changed or done anything since emailing you. The form is still not loading on anyone's computer, phone, lap top, etc. we've had our staff try it from different browsers and ip addresses. It only shows up after we refresh the page. How can this get fixed? We plan to send out a mass email tomorrow telling thousands of people to go to the page to fill out the form, but we don't want to do it when it is only randomly half working. We can postpone the email announcement until Friday but beyond that we may have to start looking at other form options that don't cause us loading issues every time.
