How to make "Configurable List" Widget Work?

We are trying to add this widget to a form.  When the white form wizard opens, we can type into the fields to try to configure it, but when we click add or try to close the form, it freezes.  We cannot get the white form wizard off the screen.  The only way to do so is to click back to the previous page in the browser.
