How and where can I put my email address in the form I have created?

I just have created a form for my website but I do not know where do I should put my email address for redirect the form to me.

One more question, I only want to create one form that is for free?

If I understand your question correctly, you want to add your email address where you want to receive submission emails. Is that correct?
