I'm not sure if I understand your query correctly. Do you mean to say to change the format of Email notification or the Thank you page? If you mean to edit the notification email, you may edit it from the Email setup wizard by clicking on Edit Email button.

The body of the email can be edited as per your requirement from the rich text editor and you can add the form fields as well.

If you are referring to Thank you page, you may also edit it from the Thank you page wizard. Please refer to the guide below that can help you with setting up Thank you page.
