Hi,
I am not sure why, but the confirmation email that we send out is not showing all the fields we selected. It pulls everything except for the Address and City fields.

As I check your JotForm, I see that there are two different address fields related to client and property. It seems that you are listing the address and city fields from the Client Info and I think it should be from the Property Info section.
If so, please replace the fields with {address} and {city} in the notification email and see if that helps. Let us know if you need any further assistance. We will be happy to assist.
