I was able to replicate mentioned issue on my cloned form.
There was an issue with "Text Format" of email Notifications two weeks ago and it seems that this is causing the issue on your form. I think that this issue was on your original form and now it is shown on your cloned form as well. To resolve this I would suggest you to delete the existing email Notification and create a new one. On my cloned form I was able to resolve this, by creating new email Notification.
