I need to create a jot form that has different categories to purchase different products. I need to have different fields saying what the product is, the price of the product, the quantity of the product and then calculate the total of the products and then grand total. I have attached a screenshot of the spreadsheet with what information that I need.

Unfortunately you cannot convert an excel spreadsheet into a form. However you can still recreate the excel form using our form builder. You can use the payment integration to fulfill your requirement with the products and its grand total.
