Page Break doesn't work

I'm trying to create a form with 2 pages. In the first page there's only a box to select an item and by clicking on NEXT button, it will open client's personal information to submit.

Here's the order:

Choice button

 - PAGE BREAK -

Name

E-mail

Adress

- SEND BUTTON -

What's wrong, then?

When I try to pre-visualize my form, it only appears the Choice Button and the Send Button. Nothing else. I've also tried to submit the form on my blogger page, and happens the same...

What am I doing wrong?

For your convenience, I have now cleared your form cache. I checked the form again and the next button is now seen in the form.
