I am getting error message "Form not found. This form is disabled."

    Hello Ahurley24,

    Upon checking your account I found that it's suspended by our anti-phishing system. As your form "SID Qualification Assessment" has form field asking for "Password" which makes it suspected of being used for phishing. Please check the screenshot below:

    Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the password label from the field and activated your account again.

    Kindly refrain from creating form with email/password, login/password or credit card details.

    I respect the sensitivity around Phisihing, however, if you explore the question is simply designed to make sure people know the password.  I can re-word.  I am assuming if I remove all will be well?

    Yes, rewording this would be acceptable.  However, asking for any sort of log in details, no matter how it is worded, is against our terms of use and will likely result in your account being suspended once again.
