We would like to apologize for any inconvenience. I have cloned your form and was able to see the issue. However, this can be fixed by removing the faulty Smooth Signature widget and re-adding another one with the same label. Please be reminded that you need to also update the conditions where that field should be included.
Here's a clone version of your form where I have applied the changes.

I checked your form and I think the issue you notice is when you try to submit the form, this is the one issue that I found in your form when tested it, I checked the web console and noticed some errors there.
To fix this I removed the Smooth Signature widget and then tried to submit the form, this worked, then I decided to add the widgets again and the form was working with the widgets on it, I would suggest you to try doing the same, remove the widgets from your form and make a submission, once you have confirmed this is working undo the changes and get the widgets back again, to do this you only need to click on the Undo button in the Form Builder:
This will bring again the widgets, and it seems like they were not properly added, I suggest you to use this option because you will not need to update your conditions again, otherwise you will need to fix those conditions where are included these widgets.
