Is it possible to have a radio button option open a new form, preferably as a popup or lightbox.
I know I can have sections that open or a page open but that is not what I want.
I have the form script embed at the site. So I was thinking it would be easier to do because of that.
I create the second form and embed lightbox script in the first form. Then I need it to appear when a particular item is selected from a radio button list.
A tricky part is, hiding the second form at some point, then adding the new form to the bottom of the first or sending at the same time when submit is pressed on the main form.

There is a new option I just added for change of ownership. When that is selected I want the new form to ask details about the custody transfer, old owner, new owner, address etc.
If this is asking to much then perhaps another option you could suggest might be better.
If I understand you correctly, you would want to have a popup window appear when a user selects a specific option. If the user in your form DogID, select YES in the question Are you a current AAC Member?. The second form would popup in a new window.
If this is not what you are looking for and did not understand it correctly, please feel free to correct me.
At the moment this is not possible as you would need to insert a script into the form. To accomplish, you would need to use the source code and add the script in the your site to create the popup window.
I thought that might be the case. As I said the form script is at our site now.
