Why my form is no longer displaying the fields?

our form here (https://form.jotform.com/50553863263962) is no longer displaying on the front end. If i add new fields or elements, they can be seen but existing form elements cannot.

i've also cloned the form, but the clone cannot be displayed either.

it seems that in the HTML, each of the items (<li>) has the the style "display: none" added to them.

Initially when checked your form I could not notice any CSS code that could be causing this, I checked the settings and nothing was there.

I finally checked the conditions and disabled all of them and then I found that this is why your form is not showing the fields.
