Despite having published my form multiple times, it will not allow me to preview the form. Furthermore, an error message appears when I use the link provided after "publishing." Please help.

This is because one of your forms has been cataloged as phishing form, this is the form:

You are asking for Credit Card number and this is something not allowed in JotForm, there is no way to collect CC numbers and save them as you do with other data, if you want to sell products, subscriptions or collect donations then you will need to use one of our payment integrations: http://www.jotform.com/help/47-Order-Form-Basics.
I had to remove this field in order to activate your account, but do not use this type of fields anymore and avoid using words such as password, pass code, username, credit card, otherwise our anti-phishing system will suspend your account, also if you are asking for Credit Card in your forms it will be disabled as well.
