Account suspended for creating phishing forms

We do not allow for the collection of usernames of passwords through our forms for any reason.  All of your forms look to be used for purposes that are not legitimate and your account will remain closed.  If you create further forms under a new account that are similar to the ones in your current account, you will likely be completely banned from the site.
