Where did all of my forms go?

I have had to replace two of them with forms from another company.  I would prefer that not to continue.  But other places on my website your forms are still linked but they are completely missing.

Originally it gave an error message for the forms that said something like "this form has been made unavailable" or something like that.  These are just information collection forms - WHY???

It's been 4 days now and I submitted an inquiry right when it was first observed and have not received a response.
