Need help ASAP please! - Submissions do not appear in INBOX

Created a donation form and the form is linked to our website ccsnj.net.  It worked fine with the first donation, however it no longer keeps the donation PDF files in the inbox.

A notification is sent, but the submissions are NOT saved in the inbox for printing.
