My forms have been disabled and I have been reading through the forum and I see that this happens when I request information not allowed. I can only consider it may be that in 2 of the 4 forms I request their paypal email which is relevant to the reason for the user submitting the form.
Can you please check, confirm and enable my forms again please or request me to take the action you require to enable my forms as soon as possible?

Looking through the Forum I now see I am not allowed to ask for usernames.
So I have edited the 2 forms which did request a username and they are now asking specifically for their (FreeMoneyClub) name. Is this allowed as it is specific and relevant to my website?
Also, when I do upgrade will it still be under TOS that usernames cannot be requested?
Regardless, please let me know if you will kindly allow my error and enable my forms again please?
If not, can you please let me know exactly what you require me to do to ensure this does not happen again.

As you already noticed, you can not ask for usernames on your forms, you can not ask for password, CC information or other sensitive data, if you would like to process payments you must use our payment integrations.
This is the reason why your account was disabled, regarding to your question about upgrading and avoid this, this is applied to all the accounts in JotForm does not matter if they are paid accounts or not, please take a look to our terms in order to know what data you should not ask on your forms: http://www.jotform.com/terms/.
Also, if you ask for data that is not allowed in JotForm your account may be suspended permanently, so please read our terms in order to avoid this happening again.
