Form not found This form is disabled.

Hello,

Upon checking your account I found that it's suspended by our anti-phishing system. Several of your form has fields like "username & Password" which makes it suspected of being used for phishing.

Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the password fields from all your forms and activated your account again.

Kindly refrain from creating form with email/password, login/password or credit card details.
