I have checked your account and your account is on Free plan. There is only one form that is shared with, from account aaronkobes. You do not have any form that is created on your account.
I also saw that your account was upgraded. It seems that wrong account was upgraded and my colleague refunded the payment on this thread.
Considering that aaronkobes is main account and form that is shared with your account is created on that account, you need to upgrade account aaronkobes to enable that account and form.
