How can I associate dynamic email address with a submit button?

I want to be able to send information from a form to an email address based on a choice made on a drop-down.  For example, if the user chooses option 1, then the information entered in a text box and the user's email entered on the email field will go to email address 1.  User chooses option 2, then info goes to email address 2 and so on.
