How to set up unique personal access keys for filling out the form

    Example, I send the form to Michael along with a personal code for Michael that will allow him to fill out the form.  It will however prevent him from forwarding the form link to one of his friends to fill out.

    It is possible to use an "access code" of sorts by using conditional logic on your form, specifically using show/hide conditions to hide all the form fields unless a correct access key is entered on the form.

    However, this alone would not be enough to prevent him from sending the link with his own access key to another person. For that, we would also need to use a Unique Question form limit, which is found under:

    Preferences > Advanced > Unique Question:

    You can set the Unique Question limit on the field where your users will be entering their access keys, so that each access key can be used in only one submission.
