Under Preferences, the status of my form is Enabled. Yet, when I try to publish the form, it says this form is disabled.

I have checked your account and found it's automatically suspended by our phishing filter. I have reviewed your form but did not find anything malicious. And for this reason, I have reactivated your account. You should be able to view now your published form.
