Forms disabled

    Greetings friends, I created just two forms have been sent to two people and now sends me a link to a page with the message "Form not available" ... they could help me with this? Thank you

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I see that your JotForm is asking for passwords which is not allowed and violating our terms of usage.

    It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on Phishing forms. 

    I have removed the guilty fields from the form and activated your account. You should be now able to see your forms now. Please refrain using such fields and remove any terms related to username or passwords immediately on your JotForms to avoid any further suspensions on your account.
