I want to integrate my from with zoho, But in the integration for, its showing only two method (contact & Lead) ?

I want to integrate my from with zoho, But in the integration for, its showing only two method (contact & Lead).

And i have another module "Potentials" where i have created all the form fields.

Please suggest how to integrate a costume form with a costume zoho module.

As per checking, it seem that only Contact and Lead are available on our Zoho integration. Can you try to integrate JotForm to Zoho using Zapier instead and see if this will help: https://zapier.com/zapbook/jotform/zoho-crm/ ?
