when I publish my form through email the link published has the information of the last patient that filled out the form

    This happens when I publish from the edit page

    This is because you have Autofill enabled. If the form was filled previously on a computer, when you re-open it on the same machine the fields will be populated with previous entries.

    If the form collect sensitive/personal information, I would suggest you disable this feature as it can be a problem if people fill the form using a public computer.
