I checked and the date field was hidden:
I cloned the form and made the field visible. I opened the Form Designer and re-sized the screen but could not replicate the same situation:
I kindly ask you to provide us more details on the circumstances when this happens and the Operating System and browser version you are using.

I know the form is hidden; what I meant was that the date is not filled in---see the date boxes are empty--as shown in your image. So, how do I make it so that those boxes keep the date?

If I am correct this happens on the following form: http://www.jotform.us/form/60965885817171
Looking at it, I see that you have set the default time, but not the default date. To set the default date you should click on the DateTime field and then click on the Default Date option in the toolbar.
Once you do, you can set it to Current to fill out the date field with the current date.
As soon as you do, it should be all set, but please do let us know how it goes :)

Yes, I did that (set the default date); in fact, I keep having to do it everynow & then! As I said, the date boxes become empty all by themselves --- I do not do anything to cause it. Is must be a glitch with either my form or with Jotform. It would be nice for the date to stay in place all the time.

I'm sorry for the trouble caused to you.
Do you mean to say that the "Default Date" is getting removed on its own? I did check your form and it does display the default date. I also sent a test submission in your form and the default date and time was captured successfully in your form's submission page.
Have you embedded your form in any webpage where you are having this issue with? Please share the webpage URL and we will take a look.
