I see what you mean after I've tested the checkboxes on your form.
I have inspected your website and I see a lot of <p> </p> in it. I understand that you have used those to create line spaces, however, those are placed in front of the embedded form that overlaps the form fields.
You can try to replace those with <br> tags.
Or, if you don't want that as an option, you can add the CSS codes to your website instead.
div.single-post.single-page p:nth-child(n+3) {
position: relative;
z-index: -1 !important;
}
div.single-post.single-page p:first-child {
position: relative;
z-index: 1 !important;
}
