I cannot update my JotForm URL to my own domain.

I am following the exact steps found at https://www.jotform.com/help/read.php?if_id=144 "How to use Custom Domain with Jotform" and I cannot figure out how to change "Jotform" to "mydomain"

I have added the CNAME to point to custom.jotform2.com but where do I go within Jotform to edit my form URL? I can only see the option to update the custom url that begins in jotform.com/...

I would like for my form to be located at quote.mpressivewebdesign.com
