I am not sure if I have understood your question correctly.
Do you mean to say that your form is not working? I did check your account and did not find any issue with that. Your account is active already and you should not have any trouble. Can you please let us know the issue you are having? Are you seeing any error message? I would suggest you to please share a screenshot of the issue and we will take a look.
