My form is not connecting to spreadsheet.

I cloned your form and tried to integrate it with google spreadsheets. It appears that I am able to integrate it to the spreadsheet successfully.

Can you please try clearing your form cache and your browser cache first, then reintegrate it again with google sheets? Here's a couple of guides that may help:
