Would like a question with more than one write-in field

I would like to make 1 question that looks like the screenshot. For this example I made three separate text questions and really it's one question with three responses. What do I click on to make a write-in question that looks like this but is only one question? I have several of them that will require this kind of response.

You can try with a Multiple Text Fields widget, or with a Matrix field.

Ah I feel like I am so close. I didn't know you could choose text boxes as opposed to radio buttons. I figured that out. Now how do I define the width of those text boxes? Right now they are very narrow and I don't see any option to change that. Please see screenshot
