So i just finish a new form for my site, and i noticed that when i was trying to Preview the form was showing as Disable, then i remove some widgets to see if the problem persist, but then i saw that all the forms in my site went Down, now all my forms are Disable, and i would like to know why?
Some help please, or answers.

You have form https://www.jotform.us/form/61585224725155 in your account that captures social security account, account username and password.
The Phishing filter suspended your account and forms.
You need to remove this form and other similar form in your account/username precich1 before we can request reactivation of the account.

Ok, is there anyway i can still request this, does this terms change on premiun supscription. because we are a Phone Service company and, well this fields are needed.

We do not allow for the collection of passwords regardless of the subscription level. This is to protect users filling out forms from phishing. Since we have no way to distinguishing between legitimate forms and phishing forms, certain sensitive data such as passwords is simply not allowed to be collected.
