can I upload my own template in a html form? (URGENT)

Hi - Can we upload a form in html on the website at all? 

We were having problems with the size of the table - it was going over the side of the actual form. even when cell with was 12.5% (8 cells wide) and table width was 100%

Please help meeeeeeeee :(

we also tried what your website advises

position:relative;

still didn't change
