This form is to be used to open a ticket in Spiceworks. The sender needs to be an email address within the form so that the ticket is opened in that users name and not email@example.com. Is this possible?

I understand that you want to use the email address field in the form as the sender email address for your notification. Unfortunately, it is not possible since the sender emails are required to configured using SMTP/verification method. It is required to use one of the email address configured in the Account settings.
