Error: form not found - this form is disabled. Why?

    I tried to view my form (https://form.jotformeu.com/61624743654358) but I can't see it. Why?

    Your account is currently autosuspended. It was suspended automatically by our phishing detector tool. I checked your account and I saw form that is asking for username and password. This is considered a phishing form. You are not allowed to collect login credentials here in JotForm. This violates our terms of use.

    In order to re-activate your account, we need to delete those fields. Let us know if that is okay with you. Thank you.

    I'm sorry for the mistake.

    Can I delete only the fields "username" and "password", without deleting all the entire form?

    I can see that you already deleted the guilty fields. I can also see that your account is now active.
