Add a "signature" to filled fields?

I am creating a form that will have multiple editors, is there a way to track who has edited what field (i.e. signature/initials)?

This would be possible by adding the appropriate fields to the form.  If you want initials, you could add smaller text boxes next to each option or we also have several signature widgets:

You could have them conditionally hidden until they are needed.
