Why is my form disabled?

    I cloned a form to make some changes and make it more appropriate for a different application and now BOTH are disabled.   Please advise why...

    I see that your account is having multiple forms collecting credit card information using plain text fields which is not allowed with JotForm. 

    It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on Phishing forms.

    If you need to collect payments using JotForm, it is required to use one of the payment tools available.

    These payment tools comply with the security standards of PCI/DSS that is required to process credit card payments. 

    It is required to agree to our terms and to delete the guilty fields from the forms. Please confirm so that we can activate your account allowing to delete the guilty fields from the form.
