and PCI DSS compliance form?

    I have a form for a client that is a simple registration for a conference, and accepting the conference fee via credit card and the integration on Jotform.

    Today, they sent me this form (see link) to complete...looks like most of the fields they can complete themselves, but some such as "payment gateway", etc... I am unsure about, would I just put "" there, or "jotform, etc.."?.


    For the shopping cart field...there really isn't a shopping cart, correct?

    Any help is appreciated. Thanks!

    I think it's best to ask what's the purpose of the PDF file. I am unsure of its usage but looks like it's a kind of a recordkeeping about the company. But for the payment gateway, you can put since the payments are all handled/process by them. You are only doing the payment form using Jotform. I think you can also mention Jotform, put Form Builder if description will be asked.

    It's payment form. We don't have a shopping cart system.

    Update: Jotform is now PCI DSS compliant. We have PCI Service Provider Level 2 certificate. We'd happy to provide certificate to any users who would need it for their payment gateway. Just contact our support team.
