why does the form not work ?

I filled the form in three times.. Tried sharing and the form kept clearing it's self. 

i tried to share an when i tried to sign in to facebook to post it crashed ?

I tried filling the form up, as well as shared it on my facebook, I made a submission and I received a Thank you page.

It's possible that your browser got clogged up with temporary data. This happens sometimes when your browser is experiencing a hard time downloading the information over the web, which results to timing out. This happens often when the internet connection of the user is slow.

What I can recommend to you is to please close all your browsers and open it again. Or at least close the tab which uses the URL and open a new one.

it the same situation with multiple visitors to my website ? can anything be done to fix ?

I tried to look into the form to see if there is anything that may be removed or any possibility which may cause the problem, but I could not find anything. The form is practically basic and it loads fast on stand alone: http://www.jotformeu.com/form/61683226056355
