It's possible that your browser got clogged up with temporary data. This happens sometimes when your browser is experiencing a hard time downloading the information over the web, which results to timing out. This happens often when the internet connection of the user is slow.
What I can recommend to you is to please close all your browsers and open it again. Or at least close the tab which uses the URL and open a new one.
If you have questions, please do let us know.
it the same situation with multiple visitors to my website ? can anything be done to fix ?
I tried to look into the form to see if there is anything that may be removed or any possibility which may cause the problem, but I could not find anything. The form is practically basic and it loads fast on stand alone: http://www.jotformeu.com/form/61683226056355
I'm not sure if this will show improvements, but you can try embedding the form using full HTML source. This guide will provide you information on how to get your form's HTML source code: https://www.jotform.com/help/104-How-to-get-the-Full-Source-Code-of-your-Form
