Form is no longer showing up after editing

    My form isn't showing up after making edits https://form.jotform.com/dfwnerdherd/dropoff

    Your account is currently autosuspended. It was suspended automatically by our phishing detector tool. I checked your forms and I saw fields that are asking for username and password. You are not allowed to collect login credentials here in JotForm. It violates our terms of use.
