Forms submitted from iPad are received blank

    Two of the yoga class registration forms submitted have been received blank. The notification arrives by email but when I click in the form it is blank. The submissions do not appear in the jotform submissions, nor the pending submissions.

    Forms should work fine no matter the device used to submit them, have you embedded the form into a mobile app? 

    This might be one of the reasons why you're receiving blank submissions since forms are made to work fine embedded on websites and using the direct links to them, embedding a form into a mobile app ( either Android or iPhone) could affect how it works. 

    However, you report that emails were received from these submissions, I checked the email log for the email address used in your form and I cannot find these blank records, there are only 2 emails sent and they are from submissions, indeed your form only has 2 submissions and 2 incomplete payments. 

    Maybe users are opening the form using the edit link instead of the link to the form, please ensure your users are opening the form using this link

    I would suggest you to remove the second button and as you're trying to add a clear fields button, please click on the submit button and enable this option on the top toolbar:
