My form on my site displays with pre-populated fields.
How to fix?
Is this some caching thing on my Chrome Browser perhaps?

I noticed that you have your form's autofill option enabled. With this setting enabled on your form, it will save the information you have filled on the fields in your browser.
You can try clearing your browser's cookies to remove the stored data when the form appears on your page.
