Hi I am trying to add my form to weebly and it keeps on saying 'form disabled'. Could you tell me how to fix this?

I have checked your account and it is currently suspended, this happens when you are asking for information that is not allowed at Jotform, then our anti-phishing detects this and proceeds to suspende the account and disable your forms. 

This happened because you named your form "Create an account" so it seems like you will ask for a password and username, this is why it is currently suspended, please try renaming your form and read our terms in order to avoid your account to be suspended again, here is the link to our terms: http://www.jotform.com/terms/

I have activated your account now and you should be able to log in, please follow this advice, otherwise your account will be permanently disabled.
