Why is my form "disabled"?

    Hello, I created a new form from a blank template today.

     

    When I finished it, I wanted to test it out, but received an error message "Form not found. This form is disabled."

     

    I have tried to "Disable" and then "Enable" this form several times, restarted JotForm, etc., and can't figure out what is causing the hangup. I've made about a dozen JotForms and never had this problem before.

     

    The only "difference" is that I used the "Smooth Signature" widget, which I've never used before.

     

    Any chance you can suggest something to make my form function?

    I have checked your form and it was suspended because of these fields:

    You're asking for Credit Card info something that is not allowed at JotForm and this is why our anti-phishing system has suspended your account and disabled your form. 

    Please take a look to our terms so you know what it is not allowed to ask at JotForm and avoid your forms to be disabled and your account to be suspended again.

    I have removed guilty fields from your form in order to activate your account so you should be able to log in now.

    I would suggest you to follow this advice, otherwise your account can be permanently deleted.
