My Preview is different from my form view

In form my label is on top and formatted in a horizontal line as I want it to be.  But the preview version shows the label in a horizontal format.  Please tell me how to correct this issue.  Thanks. 

I checked your form (both from the Form Builder and the Preview), and they look the same to me. I placed them side by side on the screenshot below for comparison:

If you're still seeing something different between the edit and preview, try logging out of your account then log back in or use a different browser to see if that will help.
