We do not allow for the collection of usernames and or passwords through forms. Two of your forms requested passwords. When our phishing detector scanned those forms, it resulted in your account being suspended.
I removed the forms that requested passwords and activated your account. You should be able to use your forms once again. Please refrain from requesting such information in the future as it will result in your account being suspended once again.
