    Why is my form disabled and not found after making a new form?

    I made a new form, and is giving me 

    Form not found
    This form is disabled.
     
     
     
    Please help!

    I believe you are likely speaking about your form titled paperbox.

    The reason your form(s) on your account have been disabled and are not found because your most recent form has violated our terms of usage

    You have fields on your form that are asking for a username, password, and bank account information which is not allowed because it is illegal and considered as phishing.

    If you wish to collect payment information then you must use one of our approved payment integrations on your form.

    I have also removed the offending fields from your form and reactivated your account so that you may continue to use it but please be aware of this in the future otherwise it may result in a permanent suspension of your account.

    Sorry I can't start a new thread because you guys have disabled my account. I notice in the answer above the reason why that might be.  On one of my forms we do request that the agent fill in their username and password for the various company platforms that we have. That is so they can have a record of what all their passwords are for each platform in one place. If we can't use JotForm to help our agents keep track of their information then we can delete that particular form. But I am hoping my account is reinstated as we were very much considering upgrading our plan with you guys. We really liked the ease of your service but we can't risk our account being suddenly deactivated without warning.
