Can you please advice why my forms are not working? What's going on?

I see that your account is auto-suspended due to the form titled "Credit Card Authorization Form" in your account. This form is collecting credit card information using plain text fields which is not allowed with JotForm. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our terms of usage for more information on phishing forms.
It is required to agree to our terms of usage and remove the guilty fields from the form in order to activate the account. Please confirm so that we can activate your account and let you delete the guilty fields from the form.
