How can I create a successful membership application form for my massage business

    Jotform has many different fields and widgets that can be added to your form to make it user friendlier.

    When reviewing the form Membership Application Form, I noticed that you have a text field asking for a signature. Instead of using the text field, you can try using one of our e-signature widgets.

    You can also use the DATE field instead of the text field to enter the date.

    How do I get my logo on the application form?

    I have a challenging time to understand the reason why you should have a signature and a signature date.

    I also would like to have this be a non-refundable monthly charge.

    How do i get this form to be on my website?

    I have a challenging time to understand the reason why you should have a signature and a signature date.

    We do not have any requirements regarding the web form fields, only you decide which fields you want to use. If you do not need the signature fields, you can simply remove them from the form.
