I am having trouble adding a personal photo to my form

I receive an error message when trying to personalize my form through designer or theme. Help?

 

 

Could you please provide us more information of the problem you are having?

What errors are you getting? What image and how you are adding it? Would you like it to use as a background image or just to display it on your live form? 

I checked the form titled "Shindy Vendor Application" and I see that the only image there is the background image.

Apologies, I forgot to mention. If you would like to display a photo or image to your form (not as a background), then simply use the "Image" field found under the "Basic Fields" section:
